aegis-defect

Record and work a post-delivery defect. Pickup gathers evidence and writes nothing; create writes the record and the story's index entry together. Attribution is proposed and evidenced, never asserted, and a done story is never reopened.

Post-delivery defects. Mode `pickup` reads the tracker item, gathers evidence and proposes an attribution - and writes nothing at all, which is what makes it safe to run on anything. Mode `create` writes the defect record and the story's `## Post-delivery fixes` entry as one act, because the validator enforces their symmetry in both directions. Mode `update` amends losslessly. Mode `handle` walks the whole path, handing off to `aegis-implement` for the fix and `aegis-review` for the check. The story stays `done` throughout - reopening it would erase the fact that it was once complete, which is the fact the defect record needs in order to mean anything.
